Original Art Intake.

Please fill out the form when receiving original art, contact Slater if you have any questions.

As you receive the art, photograph every step of unpack process including:

  1. The package before it has been opened (all sides)

  2. The open package to show how the artwork is packed inside

  3. The artwork still in its inner packaging (if applicable)

  4. Each individual artwork, front and back

  5. Any damage or potential damage to packing materials and art

If there are more than 5 pieces, fill out multiple forms until all pieces are documented.

If you take more photos than the form allows, email the whole batch to me at slaterherman@generalpublic.art.

One intake form(s) is completed, carefully put pieces into OPRA.
